What is Coronary Angioplasty (PTCA)?

An angioplasty is a surgical procedure needed to open up the blocked arteries that supply blood to your heart muscle. Why is Angioplasty Done?

Heart arteries can become blocked or narrowed from a build-up of cholesterol, cells or other substances known as plaque. This can reduce blood flow to your heart and cause discomfort in the chest. Sometimes a blood clot can suddenly form or get worse and completely block blood flow, leading to a cardiac arrest.

Angioplasty opens blocked arteries and restores normal blood flow to your heart muscle. It is done by threading a catheter (thin tube) through a small puncture in a leg or arm artery to the heart. What Is The Procedure For Angioplasty?

Angioplasty is performed by a heart specialist (cardiologist) and a team of specialized cardiovascular nurses and technicians in a special operating room called a cardiac catheterization laboratory. The following steps are followed:

    • Angioplasty is performed through an artery in the groin, arm or wrist area without general anesthesia.

Body receives fluids and blood-thinning medications (anti-coagulants) through an IV and the patient's heart rate, pulse, blood pressure and oxygen levels are monitored throughout.

  • A local anesthetic will be used to numb the area where a very small incision will be made. A small, thin guide-wire is then inserted in the blood vessel.

  • With the help of live imaging, your doctor will thread a thin tube (catheter) through your artery.

  • Contrast dye is injected through the catheter once to allow the doctor to identify the blockage (Angiogram)

  • A small balloon with or without a stent at the tip of the catheter is inflated at the site of the blockage, widening the blocked artery. After the artery is stretched, the balloon is deflated and the catheter is removed.

  • In case of several blockages, Angioplasty can take up to several hours, depending on the level of complications.

What Is Stent Placement In Angioplasty?

Most people who have an angioplasty also have a stent placed in their blocked artery during the same procedure. A stent, which looks like a tiny coil of wire mesh, supports the walls of the artery and helps prevent it from re-narrowing after the angioplasty.

Here's what happens during a stent placement:

  • At Desun Hospital, a stent (which is collapsed around a balloon at the tip of the catheter) is inserted through the wrist and guided through the artery to the blockage.
  • At the blockage, the balloon is inflated and the spring-like stent expands and locks into place inside the artery.
  • The stent stays in the artery permanently to hold it open and improve blood flow to your heart. In some cases, more than one stent may be needed to open a blockage.
  • Once the stent is in place, the balloon catheter is deflated and removed.
  • An angiogram follows, to see how well blood flows through the newly widened artery.

  • Most stents implanted during an angioplasty are drug coated. The medication in the stent is slowly released to help prevent future plaque buildup and the re-narrowing of the blood vessel.
 

Does Angioplasty hurt?

  • No, angioplasty causes very little pain. The doctor will numb the place with a local anaesthesia where the catheter will be inserted. You may feel some pressure as the stent is released.
  • You’ll be awake and alert but may be given medicine to help you relax.
