Cardiac Resynchronization Therapy Defibrillator

A Cardiac Resynchronization Therapy Defribillator or CRT-D is a cardiac device which sends out electrical impulses through insulated wires placed in the heart to coordinate the contractions of the heart’s upper and lower chambers. In addition, both ventricles are paced to improve the pumping of the heart. A CRT-D also provides therapy for life-threatening rhythms. Unlike a traditional pacemaker, a CRT-D has an extra lead on the left ventricle to make the heart pump in a more coordinated way.

CRT-D is used to treat the symptoms and other complications associated with Heart Failure (HF). HF is a common condition during which the pumping action of the heart is affected because the heart does not contract properly.

A CRT system is made up of two parts.

  • The heart device, which is actually a tiny computer, plus a battery, contained in a small titanium metal case that is about the size of a pocket watch.
  • Insulated wires, called leads, which are implanted to carry information signals from your heart to the heart device and to carry electrical impulses to your heart.

What is CRT-D?

Who needs a CRT-D?

The device monitors the heart's rhythm, detects irregularities and corrects them with electrical impulses. Like an implantable defibrillator (ICD), a CRT-D is designed to stop life-threatening ventricular tachycardia and fibrillation with relatively weak and painless stimulation therapy or electrical shocks. CRT-Ds differ from ICDs in that they help your heart’s lower chambers – the ventricles – work in tandem. To do this, CRT-Ds have three leads – wires that deliver energy to the heart.

CRT-Ds enhance effectiveness of diseased hearts and increase blood flow. They improve heart failure symptoms like fatigue, shortness of breath and exercise intolerance. Cardiac resynchronization therapy significantly lowers the risk of life-threatening cardiac events. Many patients’ quality of life improves significantly with a CRT-D. Interventional procedure is performed by a cardiologist to implant a CRT-D.

The patient may need the device if:

  • CHF (Congestive Heart Failure)
  • The patient has a medical history of or is at risk of having Ventricular Tachycardia (VT) or Ventricular Fibrillation (VF) which causes the heart to beat too fast.
  • Cardiomyopathy (weakened and enlarged heart muscle)
  • Significant electrical delay across the lower pumping chambers (bundle branch block)

Before the cardiologist decides if the patient needs a CRT-D, several investigative procedures will have to be executed to isolate the cause of arrhythmia. These could include:

  • Electrocardiogram (ECG)
  • Holter monitoring
  • Echocardiogram
  • Stress test

Readings from personal health monitoring devices, like smart watches, etc. can also help. (Ask your cardiologist if this is an option for you.)

What is the process  CRT-D Procedure ?

Arrhythmias are irregular heart rhythms and can be caused by a variety of reasons, including age, heart damage, medications and genetics. In heart failure patients CRT, or biventricular pacing, is used to help improve the heart’s rhythm and the symptoms associated with the arrhythmia.

  • The procedure to implant a CRT-D is relatively simple and usually lasts two to four hours.
  • Antibiotics are administered to reduce the risk of infection.
  • The intervention will be performed under local anesthesia, and sometimes, short-term sedation.
  • After preparing the incision site, the cardiologist will make a small cut in your upper chest, well below your shoulder.
  • Cardiologists will insert three leads into a major vein near your collarbone.
  • Using an imaging test for visibility, the cardiologist will guide the leads through the patient’s veins and place them in the heart’s upper and lower chambers.
  • Then, the cardiologist will implant the CRT-D in a pocket created at the incision beneath the skin.
  • Cardiologists will connect the leads to the defibrillator and program it to the specific medical needs.
  • After a test to ensure that the CRT-D is working correctly, the cardiologist will close the incision with a few stitches.

The patient will usually stay overnight in the hospital after cardiac resynchronization therapy. The cardiologist will test the device to make sure it's programmed correctly before the patient leaves the hospital. Most people can return to their usual activities after a few days.

Having a CRT device should improve symptoms caused by slow heartbeat, such as fatigue, lightheadedness and fainting.

Why it is used?

The device monitors the heart's rhythm, detects irregularities and corrects them with electrical impulses. Like an implantable defibrillator (ICD), a CRT-D is designed to stop life-threatening ventricular tachycardia and fibrillation with relatively weak and painless stimulation therapy or electrical shocks. CRT-Ds differ from ICDs in that they help your heart’s lower chambers – the ventricles – work in tandem. To do this, CRT-Ds have three leads – wires that deliver energy to the heart.

Cardiac resynchronization therapy significantly lowers the risk of life-threatening cardiac events. Many patients’ quality of life improves significantly with a CRT-D. Interventional procedure is performed by a cardiologist to implant a CRT-D.

When will I need a CRT-D?

What are the benefits of CRT-D implantation?

The main benefit of a CRT-D is that it constantly senses the heart’s rhythm and automatically treats an arrhythmia which may be associated with your heart failure diagnosis.

How do I live with my CRT-D?

  • Check your pulse and keep a record of it the way your doctor tells you.
  • Your doctor will check your device every three to six months.
  • Take your medicine the way your doctors tell you.
  • Follow all doctors’ advice..
  • Carry an I.D. card with you so others know you have an ICD.
  • Tell airport security that you have an ICD.
  • Household microwaves, electric appliances, most office and light shop equipment will NOT affect your ICD.
  • If you work around industrial microwaves, electricity, cars or other large motors, ask your doctor about possible effects..

Can I use a mobile phone if I have an CRT-D?

You can use a cell phone, if you take these steps:

  • Hold the phone to the ear on the side of your body opposite from your ICD.
  • When your phone is on, try to keep it at least six inches away from the device. For example ,Don’t carry your phone in your chest pocket over your device.
