Mitral valve repair and Mitral Valve Replacement (MVR) are procedures that are performed to treat diseases of the mitral valve — the valve located between the left heart chambers (left atrium and left ventricle). If the patient has mitral valve disease, they may eventually need mitral valve repair or mitral valve transplant surgery to treat the condition.
Several types of mitral valve disease exist. Treatment for mitral valve disease depends on the severity of the patient’s condition. Doctors may recommend surgery to repair or replace mitral valves for some people with mitral valve disease. Several surgical procedures exist to repair or replace mitral valves, including open-heart surgery or minimally invasive heart surgery.

Mitral valve stenosis and mitral valve regurgitation (also known as mitral insufficiency) are two different types of problems that might need valve replacement.

In mitral valve stenosis, the valve is unable to open fully, and less blood is able to move from the left atria into the left ventricle. The leaflets become thick or stiff, and they may fuse together. This results in a narrowed valve opening and reduced blood flow through the valve.
In mitral valve regurgitation, the valve is leaky. Some blood leaks back into the left atrium instead of moving forward into the left ventricle. In mitral valve regurgitation, the flaps (leaflets) of the mitral valve don’t close tightly, and this condition commonly occurs due to valve leaflets bulging back — a condition called mitral valve prolapse.
These valve problems can lead to symptoms as fatigue and shortness of breath. If these symptoms become severe, surgery may be necessary.

Both mitral stenosis and mitral regurgitation can result from general aging of the valve. Other causes of mitral valve disease include:

Ischemic heart disease (coronary artery disease)
Infection of the heart valves
Heart failure
Rheumatic fever

The doctor may first suggest monitoring the patient’s condition with regular evaluations if the patient is not experiencing symptoms or if the condition is mild. Medications may be prescribed to manage symptoms. If the condition is mild, the patient may not need surgery.

However, the mitral valve may eventually need to be repaired or replaced. In some cases, doctors may recommend mitral valve repair or mitral valve replacement even if the patient is not experiencing symptoms. Research has found that performing surgery in a person with severe mitral valve regurgitation who isn’t experiencing symptoms, rather than monitoring the condition, can improve long-term outcomes.

If the patient needs heart surgery for another condition in addition to mitral valve disease, doctors may conduct surgery to treat both conditions at the same time.

The patient may need some routine tests before the procedure to assess overall health before surgery. These may include:

Chest X-ray
Electrocardiogram (ECG)
Blood tests
Echocardiogram
Coronary angiogram (to assess blood flow in your heart arteries)

If the patient’s mitral valve can’t be repaired, the doctor may recommend mitral valve replacement.

In general, during open mitral valve replacement:

The surgery is done under general anesthesia and usually lasts 4 to 6 hours.
The doctor makes an incision down the middle of the patient’s chest. To access the heart, the doctor separates your breastbone.
The surgery team attaches the patient to a heart-lung machine. This machine acts as the heart and lungs during the procedure.
The surgeon then removes the mitral heart valve and replaces it with a new valve.
The surgery team then removes the heart-lung machine.
The team finally wires the breastbone back together.
The patient generally spends 2 – 3 days in the cardiac care unit (CCU) where they are given fluids, nutrition and medications through intravenous (IV) lines. Other tubes drain urine from the bladder and drain fluid and blood from the heart and chest and the patient may also be given oxygen.

The treatment team monitors the patient’s condition and watches for signs of infection in the incision sites. All vitals are checked frequently.

The treatment team may instruct the patient to walk regularly to gradually increase activity and to do breathing exercises as they recover.

The doctor gives instructions to follow during recovery, such as properly caring for incisions, taking medications, and managing pain and other side effects after surgery.

Recovery time depends on the procedure, overall health before the procedure and any complications. The doctor may advise the patient to avoid driving a car or lifting anything more than 10 pounds for several weeks and instruct the patient about when he/she can return to normal activities.
